Most people don't think about Medicare until it's three months away. By then, the alphabet soup of Parts A, B, C, and D — plus enrollment windows, premium penalties, and a dozen plan choices — feels overwhelming. This guide is what we wish every family knew six months before they needed to make decisions.
The four parts of Medicare, explained once
Medicare is divided into four parts. They're not levels — they're categories of coverage, and most people end up using a combination.
Part A — hospital insurance
Covers inpatient hospital stays, skilled nursing facility care, hospice, and some home health care. Most people get Part A premium-free at 65 because they (or their spouse) paid Medicare taxes during their working years. If you didn't work the full required quarters, there's a monthly premium, but most people pay nothing.
Part B — medical insurance
Covers doctor visits, outpatient care, preventive services, durable medical equipment, and some home health care. Part B has a monthly premium that's higher for high-income earners through IRMAA — the Income-Related Monthly Adjustment Amount. It's deducted from your Social Security check if you're collecting benefits.
Part C — Medicare Advantage
Not really a separate "part" of Medicare — it's a private alternative that bundles Parts A, B, and usually D into a single plan, run by insurance companies under contract with Medicare. Premiums vary from plan to plan, plans are restricted to a provider network, and many add dental, vision, hearing, and gym memberships. We cover this in depth on our Medicare Advantage page.
Part D — prescription drug coverage
Covers prescription medications. Available as a standalone Part D plan added to Original Medicare, or built into a Medicare Advantage plan (called MAPD). Premiums vary depending on the plan you pick.
Original Medicare vs. Medicare Advantage
This is the most important decision most families face. There are essentially two paths:
- Path 1 — Original Medicare (Parts A and B) + a Medigap policy + a standalone Part D plan. Higher monthly cost, total freedom of providers, predictable out-of-pocket costs.
- Path 2 — Medicare Advantage (Part C, usually with built-in Part D). Lower monthly cost (varies by plan), provider network restrictions, extra benefits like dental, vision, and hearing.
There is no universally "better" path. The right choice depends on your doctors, your prescriptions, your travel habits, your budget, and your honest sense of how your health may change. The enrollment windows you cannot miss
- Initial Enrollment Period (IEP): your seven-month window around your 65th birthday — three months before, the month of, and three months after. Wait past it without other creditable coverage and you'll pay late enrollment penalties for the rest of your life.
- Annual Enrollment Period (AEP) — October 15 to December 7: every existing Medicare member can switch plans, change carriers, or move between Original Medicare and Medicare Advantage. Changes take effect January 1. This is the time to re-shop.
- Special Enrollment Periods (SEPs): life events open windows year-round — moving out of your plan's service area, losing employer coverage, becoming eligible for Medicaid, qualifying for Extra Help. If your life has changed materially, there's likely an SEP that applies.
The three big mistakes families make
1. Delaying Medicare because of a misconception
The most common: "If I sign up for Medicare, I'll lose Social Security." Not true. Medicare and Social Security are separate programs. Your Part B premium gets deducted from your Social Security check, but your benefit doesn't decrease. Delaying Medicare to "protect" Social Security costs people thousands in late enrollment penalties.
2. Picking a plan based on premium alone
A low-premium Medicare Advantage plan can quietly cost you thousands if your doctors aren't in-network, or your prescriptions land in expensive formulary tiers. A "low" Plan G premium can be the wrong choice if the carrier has a history of large annual rate increases. The right Medicare decision compares total annual cost, not just monthly premium.
3. Not re-shopping every year
Plans change every year — premiums, drug formularies, provider networks, supplemental benefits. The plan that was perfect two years ago may be the worst plan available to you today. Free annual reviews exist precisely because the answer keeps changing. Use them.

If you have employer health coverage that's considered "creditable" by Medicare, you can defer Medicare without penalty. When you eventually leave employer coverage, you'll have a Special Enrollment Period to enroll without late fees. Important: enroll in Part A anyway — it's usually premium-free and provides supplemental hospital coverage. Just don't enroll in Part B until you're ready to start paying its premium.
If you're working past 65 with employer coverage, talk to an agent before retirement to plan the transition. Get your employer to provide a Notice of Creditable Coverage — that's the document that proves you don't owe late enrollment penalties.
How to use this guide
- Six months before turning 65: get familiar with the basic structure (this article). Decide roughly whether you're leaning Original Medicare + Medigap or Medicare Advantage.
- Three months before: your Initial Enrollment Period opens. Talk to an independent agent. Run real quotes against your specific prescriptions and doctors. Make a decision.
- Month of your 65th birthday: enroll. Coverage starts the first of the month you turn 65 if your IEP began on time.
- Each fall during AEP: have someone review your plan against new options. It takes 30 minutes and often saves a meaningful amount each year.
One last thing
Medicare is one of the few decisions where the wrong choice can quietly cost you thousands a year for the rest of your life. It's also one of the most navigable insurance decisions you'll ever make — if you have someone in your corner who will sit down with you and explain it without trying to sell you the plan that pays them the highest commission.
