What "dual eligible" actually means
Dual eligible means you qualify for Medicare and Medicaid at the same time. When that happens, Medicare becomes your primary coverage and Medicaid steps in to help with what Medicare doesn't fully cover — which can include your premiums, deductibles, copays, and services Medicare largely leaves out, such as long-term care.
Millions of Americans are dual eligible, and a striking number don't know it. Qualifying for Medicaid doesn't come with a letter explaining which Medicare plans that unlocks, and qualifying for Medicare doesn't prompt anyone to check whether you'd also qualify for Medicaid. The two programs are run by different agencies, and the gap between them is where people lose benefits they're entitled to.
D-SNP plans: built for this exact situation
A Dual Eligible Special Needs Plan (D-SNP) is a type of Medicare Advantage plan designed only for people with both Medicare and Medicaid. Instead of managing two programs that don't talk to each other, a D-SNP coordinates both under one plan and one card.
Because Medicaid is already covering much of your cost sharing, D-SNPs can direct their budget toward benefits traditional Medicare doesn't offer. Depending on the plan and your county, that commonly includes:
- Dental, vision, and hearing coverage — the three things people are most surprised Original Medicare doesn't include.
- Transportation to and from medical appointments.
- An over-the-counter or grocery allowance — a set amount each month or quarter for approved items.
- Care coordination — a case manager who helps organize your providers and benefits.
- Plan premiums that vary, on top of the Medicare costs Medicaid is already helping with.
Benefits vary significantly by plan, carrier, and county — this is not a product where one plan is best for everyone, and that's precisely why comparing them matters.
Who qualifies for Medicaid in Virginia
Virginia expanded Medicaid, and the program is administered under the name Cardinal Care. In broad terms:
- Adults under 65 generally qualify with household income up to 138% of the federal poverty level. Eligibility at this level is based on income, not assets.
- Adults 65 and older, and people who are blind or have a disability, fall under different rules that consider both income and assets, with separate limits.
- Long-term care Medicaid — coverage for nursing facility care or home and community-based services — has its own eligibility standards, generally stricter and asset-tested.
Income and asset limits are adjusted annually, so any specific dollar figure you read online goes stale quickly. The determination is made by the Commonwealth, not by an insurance agency — you apply through CoverVA or at coverva.dmas.virginia.gov. Extra Help and Medicare Savings Programs — check even if Medicaid says no
This is the part we most often find people missing. There are programs that help with Medicare costs at income levels above full Medicaid eligibility, and they're chronically underused.

Qualifying for a Medicare Savings Program often also qualifies you automatically for Extra Help, and either can open the door to a D-SNP. If someone once told you that you "make too much for Medicaid," that answer says nothing about these programs. It's worth a second look.
If you don't qualify for Medicaid
Not qualifying isn't a dead end, it just changes the path. If you're under 65 and above the Medicaid threshold, an ACA marketplace plan with premium tax credits is usually the next step, and those subsidies often cut the cost dramatically — many people who assume marketplace coverage is unaffordable have never seen their actual subsidized price. One genuine advantage of dual eligibility: you generally aren't locked into the standard Medicare calendar. Most people can only change Medicare Advantage plans during the Annual Enrollment Period each October 15 – December 7. People with both Medicare and Medicaid typically have a Special Enrollment Period that allows plan changes more often during the year — commonly once per quarter for the first three quarters, though the specific rules depend on your situation and level of Medicaid eligibility.
That flexibility matters. If you're in a plan that isn't working — your doctor left the network, your medication isn't covered well — you may not have to wait until fall to fix it.
